rework state and ops, store descriptor once

The state stores the descriptor in multiple places. While this should be references to the same copy of the descriptors, we can simplify this site a bit:

• symbols reference a filename and not a descriptor
• filenames reference a descriptor

This ensures we only have one copy of each descriptor, unlocking future efforts to combine some descriptors
