When I first open Carla with an empty session, Carla itself does not appear in the patchbay window. Other programs and devices do. If I click "Configure Carla" and then just press OK without changing anything, Carla appears in the patchbay window. I'm using version 2.4.3 on Linux Mint 20.3, with JACK as the audio driver running on Pipewire 0.3.51.

what do you mean by "appear in the patchbay window"?

When the "Patchbay" tab is selected at the top, as shown here:

Once I've gone into settings and pressed OK, a new "block" appears on that screen for Carla.

You have no plugins loaded on that screenshot, so makes sense for Carla to not be there, there is nothing to show.
Unless you run in Rack mode, that will be the only mode where it makes sense for Carla to appear.

Carla doesn't appear even when I do load a plugin into the rack. I can only make Carla appear by going into the settings and back out. This means I have to do this whenever I want to route Carla's audio to the sound card. As for rack mode, the settings are set for "continuous rack". I'm not sure if that matters, but surely changing no settings and then clicking OK shouldn't make a difference?

You can do canvas->refresh for that, no?

best to try in jack2 to compare, this might just be a pipewire bug.

Yes, Canvas->Refresh does make Carla appear in the patchbay as well. It works regardless of whether any plugins are loaded; if I refresh right after startup, Carla appears too.
