At least on my end the steps to reproduce is:

  1. Log into twitter
  2. Register on my site using twitter
  3. Log out of my site
  4. Log out of twitter
  5. Log into my site using twitter
  6. Twitter gives a prompt to both login and authorize and I authorize

When twitter sends me back to my site I get:

No access token saved for "twitter.com".
