  1. Add support for SCSS and custom CSS for different formats.

    A print PDF can have a different styling, like showing link urls or
    acronym's definition. You may also want to style your HTML for browser,
    so all these cases are now possible.
  2. Add ERb support.

  3. Only support redcarpet with rouge as syntax highlighter.

    This commit drops support for:
    - pygments.rb
    - files other than .md
    - other markdown libraries
    - custom syntax highlight with @@@ anotation

  1. Now markup files can be created outside a chapter folder.

    Bug fix: Textile footnote extension wasn't considering breaklines.
    Now the syntax.css file contains all syntaxes; easier to change theme.
