ATRIA Bleeding Risk Score

Description

  • A risk stratification score to predict warfarin-associated hemorrhage and guide physician decision-making regarding warfarin use in atrial fibrillation. This hemorrhage risk stratification scheme is based on the ATRIA (Anticoagulation and Risk Factors in Atrial Fibrillation) study and classifies individuals with atrial fibrillation as low, intermediate or high risk of major hemorrhage (fatal OR requiring ≥2 U of packed red blood cells OR hemorrhage into a critical anatomical site e.g intracranial or retroperitoneal). Points are assigned based on the presence of the following 5 easily assessable clinical variables: anemia = 3 points; severe renal disease (estimated glomerular filtration rate <30 ml/min or on dialysis) = 3 points; age ≥75 years = 2 points; prior bleeding = 1 point and hypertension = 1 point, with a minimum = 0 and a maximum = 10. Individuals with total score <4 are at low risk (0.8%) of warfarin-associated hemorrhage, score of 4 is intermediate risk (2.6 %) and score >4 is classified as high risk (5.8%). It has been validated as simple and effective particularly in predicting external hemorrhage risk, but is out-performed by HAS-BLED score, particularly in predicting risk of internal hemorrhage.

ATRIA Stroke Risk Score

Description

  • ATRIA (anticoagulation and risk factors in atrial fibrillation) stroke risk score is used to determine the risk of stroke in patients with atrial fibrillation. It is the sum of points assigned for the presence (or absence) of certain risk factors: previous stroke, age, sex, comorbidities (diabetes, congestive heart failure, hypertension), proteinuria, and eGFR < 45ml/min (or end stage renal disease) in the patient. The ATRIA stroke risk score takes a value between 0 - 15, and it is from this score that ATRIA stroke risk is predicted. A score between 0 - 5 predicts a risk of ischaemic stroke/TE less than 1%; a score of 6 predicts 1 - <2% risk; and a score >6 predicts ≥2% risk.

CHA2DS2-VASc Score

Description

  • Risk stratification for stroke based on the CHA2DS2-VASc score defined by the European Society of Cardiology (ESC) guidelines for management of atrial fibrillation. The result is interpreted as 0p - low risk, 1p - intermediate risk, 2p or more - high risk. For specific risk rates, please view model description or reference [4].

Corrected QT Interval (QTc) Calculator

Description

  • Calculates the corrected QT interval (QTc), which is an estimate of the QT interval at a heart rate of 60 beats per minute (bpm). QTc allows comparison of QT values over time at different heart rates and improves detection of patients at increased risk of arrhythmias. QTc is expressed in milliseconds and is estimated using the heart rate (bpm) and QT interval (milliseconds) as variously expressed by the Bazett, Fredericia, Framingham and Hodges formulae. QTc is prolonged if > 440ms in men or > 460ms in women, and associated with increased risk of torsades de pointes if > 500ms in any sex. QTc is abnormally short if < 350ms.

Corrected Calcium

Description

  • Corrected calcium is the total calcium concentration in an individual when adjusted for the presence of hypoalbuminaemia in that individual. Because serum calcium (part of total calcium concentration) is principally bound to the plasma protein albumin, when levels of albumin are low it results in a falsely low total calcium concentration. The Payne formula performs the necessary adjustments to determine the actual total calcium concentration in individuals with hypoalbuminaemia: corrected calcium = serum calcium + 0.8 * [(40 - serum albumin) / 10]. Serum calcium is measured in mg/dl, serum albumin is measured in g/L and 40 in the equation refers to 40 g/L which is the assumed population normal for albumin.

Description

Description

Description

Glasgow-Blatchford Bleeding Score

Description

  • A risk-stratification system for patients with acute upper-gastrointestinal bleeding (UGIB), to discriminate between patients at high or low risk of dying or rebleeding. The score is calculated based on clinical and laboratory variables only. A score above 0 (zero) implies the patient is at high risk of needing clinical intervention (transfusion, endoscopy or surgery), and correlates well with length of hospital stay and number of units of blood transfused. GBS is more sensitive than clinical and complete Rockall scores for identifying high-risk patients with acute UGIB.

Pulmonary Embolism Severity Index (PESI)

Description

  • Pulmonary embolism severity index (PESI) is a risk-scoring system used to predict the 30-day outcome (mortality and long-term morbidity) in individuals with pulmonary embolism (PE) using 11 non-laboratory based characteristics; including 2 demographic (age, male sex), 3 comorbid (cancer, heart failure, chronic lung disease) and 6 clinical (pulse ⩾ 110/min, systolic blood pressure < 100 mmHg, respiratory rate ⩾ 30/min, temperature < 36°C, altered mental status [disorientation, lethargy, stupor, or coma], arterial oxygen saturation < 90%). Points are assigned based on the presence of each characteristic and the sum of the 11 points assigned is used to categorize patients into risk classes: class I (score 0 - 65) is very low risk, class II (66 - 85) is low risk, class III (86 - 105) is intermediate risk, class IV (106 - 125) is high risk and class V (> 125) is very high risk. These classes correspond to 30-day mortality of 0.0-1.6%, 1.7-3.5%, 3.2-7.1%, 4.0-11.4% and 10.0-24.5% respectively. PESI has been internally and externally validated as having high sensitivity for predicting mortality and reliably identifies patients with PE who are at low risk of death and who are potential candidates for outpatient care.

Model For End-Stage Liver Disease Score (MELD)

Description

  • Model For End-Stage Liver Disease (MELD) score is a measure of mortality risk in patients with end-stage liver disease. It is a reliable way of estimating disease severity, determining prognosis, and assigning organ allocation priority in patients with end-stage liver disease. It is the standard used by the Organ Procurement and Transplantation Network (OPTN) and determines who is the highest priority to receive liver transplants in the US. The score is calculated from a regression equation that includes serum sodium, creatinine and total bilirubin concentrations, the International Normalized Ratio for prothrombin time (INR) and whether or not the individual has had at least two dialysis sessions in the last week before serum creatinine was measured. It is based on a previous "Original MELD score" that is calculated using the same equation and the same set of variables, but does not include serum sodium concentration. MELD score is considered as good as the Child-Turcotte-Pugh (CTP) score in predicting short-term mortality in end-stage liver disease, as well as an accurate predictor of survival in patients with variceal bleeding, mortality in patients with cirrhosis who develop infections, mortality in patients with fulminant hepatic failure, alcoholic hepatitis and other forms of chronic liver disease.

Fractional Excretion of Sodium (FENa)

Description

  • Calculates the fractional excretion of sodium (FENa) which is the percentage of the sodium filtered by the kidney which is excreted in the urine. FENa helps distinguish between prerenal, intrinsic renal and postrenal causes of renal failure, and is widely used to differentiate prerenal disease (decreased renal perfusion) from acute tubular necrosis (ATN) as the cause of acute kidney injury (AKI). The calculation requires serum (sNa) and urine (uNa) sodium concentrations (mmol/L), and serum (sCr) and urine (uCr) creatinine concentrations (umol/L or mg/dl). FENa < 1% and uNa < 20mmol/L suggests kidney injury of prerenal etiology; FENa > 1% and uNa > 40mmol/L suggests an intrinsic renal etiology; while FENa > 4% and uNa > 40mmol/L suggests a postrenal etiology.

Fractional Excretion of Urea (FEUrea)

Description

  • Fractional excretion of urea (FEUrea) is the percentage of blood urea nitrogen filtered by the kidney which is excreted in the urine. FEUrea helps distinguish between prerenal and intrinsic renal causes of acute renal failure, specifically prerenal azotemia and acute tubular nerosis (ATN). Though less common than fractional excretion of sodium (FENa), it is more specific and sensitive and potentially more useful because urea excretion (unlike sodium) is not significantly affected by diuretics. Pediatrics

Asthma Predictive Index (API)

Description

  • Asthma predictive index (API) applies to children/infants ≤3 years old and is a method for predicting likelihood of a later diagnosis of childhood asthma. It is based on the presence of at least one of 2 major criteria and at least two of 3 minor criteria, plus the number of wheezing episodes per year. Major criteria are: parent with asthma/patient with eczema; minor criteria are: patient with allergic rhinitis/wheezing apart from colds/eosinophilia (≥ 4% on FBC). A positive API is fulfilled by either 'strict index' (≥3 episodes of wheezing per year, AND ≥1 major criteria OR ≥2 minor criteria) or 'loose index' (<3 episodes of wheezing per year, AND ≥1 major criteria OR ≥2 minor criteria), and they each provide >95% specificity, and 80% specificity respectively. It has good specificity but is not a good screening tool and does not detect many patients who will later be diagnosed with asthma. It allows for more aggressive trials of asthma medications in patients who are likely to be diagnosed with asthma later in life.

Dutch Lipid Clinic Network Criteria (DLCN)

Description

  • The Dutch Lipid Clinic Network (DLCN) criteria are a set of 10 questions recommended by the European Atherosclerosis Society (EAS) for aiding the diagnosis of heterozygous Familial Hypercholesterolaemia (FH) in adults. The questions fall into 5 groups: family history (first degree relative with premature coronary heart disease, tendon xanthoma, corneal arcus, or LDL cholesterol >95th percentile); clinical history (premature coronary heart disease, or premature cerebral or peripheral vascular disease); physical examination (tendon xanthoma, or corneal arcus); biochemical results (LDL cholesterol); and molecular genetic testing (DNA analysis). Presence of each criterion carries a specific number of points, but only the highest scoring criterion present is selected from each group to contribute to the final DLCN score; minimum score = 0 and maximum score = 26. DLCN score >8 is ‘definite FH’, score 6 - 8 is ‘probable FH', score 3 - 5 is ‘possible FH’ and score 0 - 2 is ‘unlikely FH’.

Description

  • Peak expiratory flow (peak expiratory flow rate, PEFR) is the maximum flow rate of air generated during forceful exhalation from fully inflated lungs. Drug Use Disorders Identification Test (DUDIT)

Description

  • Drug use disorders identification test (DUDIT) is a screening test for identifying individuals with drug-related problems, those in the risk zone, and those who clearly do not have drug-related problems. The test consists of 11 items or psychometric indicators which help assess drug-use patterns and various drug-related problems. The first 9 items are scored 0, 1, 2, 3 or 4, while the 10th and 11th items (assessing "harmful use" and "concern from others" respectively) are scored 0, 2 or 4. Minimum total score = 0 (no drug-related problems) and maximum score = 44 (extremely dependent on multiple drugs).

Montgomery Åsberg Depression Rating Scale (MADRS)

Description

  • The purpose of the test is to give a detailed assessment of a patients current state of mind, that can be used as a tool to evaluate depression and suicidal tendencies. The rating should be based on a clinical interview moving from broadly phrased questions about symptoms to more detailed ones which allow a precise rating of severity. The interviewer must decide whether the rating lies on the defined scale steps (0, 2, 4, 6) or between them (1, 3, 5) and then report the appropriate number. Description

  • Yale-Brown obsessive compulsive scale (Y-BOCS) is a 10-item self-reported questionnaire used to assess the severity of symptoms, as well as monitor treatment improvement in patients with obsessive compulsive disorder (OCD). 5 items measure obsessions and 5 items measure compulsions, all in terms of the time occupied, how much they interfere with functioning, the patient’s degree of distress, and his or her attempts to resist symptoms and the ability to control them successfully. Each item is rated 0 = no symptoms to 4 = extreme symptoms, yielding a minimum possible score = 0 and a maximum possible score = 40.

Acute Gout Diagnosis Rule

Description

  • An easy to use diagnostic rule for determining whether a patient's symptoms are due to acute gouty arthritis, and if there is need for further laboratory testing. The assessment is a clinical one that is made without joint fluid analysis. Points are assigned for each of 7 variables: sex, previous report of arthritis, onset within 1 day, joint redness, involvement of 1st metatarsophalangeal joint, cardiovascular disease and serum uric acid level. A total score <=4 is unlikely to be gout (gout prevalence 2.2%), score >4 and <8 is indeterminate (gout prevalence 31.2%), and a score >=8 is likely to be gout (gout prevalence 80.4%). Synovial fluid analysis of the affected joint for monosodium urate (MSU) crystals is recommended in the indeterminate group.

Trauma Care

ABC Score for massive transfusion

Description

  • Assessment of Blood Consumption (ABC) Score is used for early and rapid estimation of the need for massive transfusion in acutely injured patients, using four simple clinical and bedside criteria: penetrating mechanism of injury, arrival systolic blood pressure of 90 mmHg or less, arrival heart rate of 120 bpm or greater, and positive FAST (focused assessment with sonography in trauma). The presence of any variable contributes one point to the total score giving a minimum score of 0 and a maximum possible score of 4. A score of 2 or greater is likely to require massive transfusion. ABC score was the simplest and the quickest, but least accurate (13.1%), in an efficiency assessment of massive transfusion scoring systems, when compared with trauma associated severe haemorrhage (TASH) and emergency transfusion score (ETS).

Injury Severity Score (ISS)

Description

  • Anatomical based scoring system for numerical description of the severity of injury in persons who have sustained trauma to mutiple body regions. ISS = AA + BB + C*C, where A, B and C are the AIS (abbreviated injury scale) scores assigned to the most severe injuries in the three most severely injured body regions (head/neck, face, chest, abdomen, extremities and skin). Possible AIS scores range from 0 = no injury to 5 = critical injury, while possible ISS ranges from 0 - 75. An unsurvivable injury (AIS score of 6) in any body region results in the individual automatically assigned an ISS of 75 - an injury incompatible with life. ISS shows linear correlation with hospitalisation, morbidity and mortality following trauma.

Revised Trauma Score (RTS)

Description

  • Revised Trauma Score (RTS) is a physiologic score of injury severity performed at first patient contact following trauma, and it numerically summarizes assessments of circulatory (Systolic Blood Pressure), respiratory (Respiratory Rate) and central nervous system (Glasgow Coma Scale score) function, using coded and weighted forms of the three physiologic variables. It has high inter-rater ability and is used to quantify the severity of injury, to predict survival/death outcome and (with unweighted forms of the variables) as a triage tool in the pre-hospital setting (Triage-RTS). RTS is scored from 0 - 7.8408 (includes non-integer values), and higher RTS scores are associated with greater probability of survival. T-RTS is scored from 0 - 12 (only integer values), and patients with T-RTS = 1 - 10 (T1) require immediate life-saving intervention; T-RTS = 11 (T2) require urgent intervention within two to four hours, T-RTS = 12 (T3) can have delayed care, while T-RTS = 0 implies a dead patient.
