Include uppercase C and H as C++ extensions #445

Some projects (for example, opensource CFD package OpenFOAM) use uppercase C and H as extensions for C++ files (and these extensions seem to have been popular in early years of C++). It would be convenient to include these extensions into --cpp preset.

Thanks for the suggestion. It's fixed in master now, and will be in the next release.
