Remove inert, h2o2, and vision from core

In an effort to keep core simpler and smaller, this will remove the dependencies on the three plugins. To use them, you will need to manually require them and register them with the server. This will allow for faster releases on the plugins as well as create the opportunity to provide alternative implementation for the functionality they provide.
