Password constantly being asked for

Every log in and every file transfer I am being asked for password even though add to keychain is checked.

Can you find the password stored in Keychain Access.app?

The login is in keychain however it says that the password is restricted. I have attached a screen shot.

Something is wrong with that keychain. I suggest trying to repair it using Keychain Access.app → Keychain First Aid.
