Fluent client should ignore null/empty search params #150

Per an email from Al Amoyt on the list

The fluent style is very elegant but any particular call is 'hard coded'. Can some of these be null or empty and simply ignored by the FHIR client?

For example, a user interface allows a client search on name, DOB and gender but any of these can be left 'empty' making for 8 combinations . Can I could code one fluent call with all the criteria and have it ignore those that are 'empty'? Of course, we need to know how 'empty' is defined' (a null object reference?)
