Non-unique table header names causes corrupt file #362

If a XlsxWriter table contains non-unique headers it leads to a corrupt file. Excel requires unique, case-insensitive headers.

Sample program that demonstrates the issue:

import xlsxwriter 

workbook = xlsxwriter.Workbook('test.xlsx')
worksheet = workbook.add_worksheet()

worksheet.add_table(0, 0, 1, 1, {'columns': [{'header': 'ID'},
                                             {'header': 'ID'}]})
workbook.close()
