Dialog custom select doesn't report value to button #2181

On HEAD attempting to change the value of the "your state" select menu results in no change.

Refactor to follow to centralize the bind. It looks to me like a lot of this should go in the page plugin be?

The main difference I see with a select-menu dialog is you now get the loading screen after attempting to close. The parent page appears to still be in the dom, but there is a perceived lag.
