Regarding the data-add-back-btn attribute ...

http://demos.jquerymobile.com/1.4.0/toolbar/ states:

This can also be set via markup if the page div has a data-add-back-btn="true" attribute.

http://api.jquerymobile.com/toolbar/ states:

This can also be set via markup if the header div has a data-add-back-btn="true" attribute.

It seems the API Documentation is correct in stating the header div is the correct location for the attribute.
