When running Set up TOTP a additional KPH attribute for {TOTP} is created for browser integration if it doesn't already exist.

Wouldn't be better to just skip the creation of this attribute and allow the extension to get the TOTP without it?

PR updated and rebased with the simplest possible solution. Related browser PR is keepassxreboot/keepassxc-browser#119 which supports also the old KHP attribute format (for now).
