Update Slack communication guidelines #3398

This restructures the slack guidelines and adds an issue template for Slack requests.

The goal is to move channel and bot/token/webhook requests to github where they may be discussed in a more async manor and get more eyes on the subject.

I would stick with "will", but remove the "automatically" part. Admins will do their best to remove any content that violates the CoC.
