just tell me if there are indications or guide lines to follow and what is your preferred way to have it... a fork in which I create a README.it.md from which to open a PR would suffice?

Just fork and create README.it.md
then after translating this version just Pull request.

How you think to manage updates in future?

I was referring to the way translators will be notified of updates on the original file ...

How to have translations in sync?
