Add support for reconstructing tiles from lower levels #63

The maximum cached level for a given <tileset> is specified
for each grid. E.g. to activate this, use

<tileset>
  <grid max-cached-zoom="12">WGS84</grid>
  <!-- ... -->
</tileset>

This will cause all tiles above level 13 to be created
at request time by resampling from the tiles in level 12.

cc @martinchristen : support for out-of-zoom tiles by proxying the source instead of reconstructing from lower level tiles is included in this pull-request. The actual proxying is yet to be implemented, c.f. https://github.com/mapserver/mapcache/pull/63/files#L4R709
