Relating to a Twitter exchange (https://twitter.com/skram/status/379007453830316032), there are a number of NPIs with data anomalies that we have found throughout this project:

• 1366556300 (http://www.bloomapi.com/search#/npis/1366556300) practice location has a AZ zip code and area code but a state of IN
  • I think IN should be AZ
• 1174629083 (http://www.bloomapi.com/search#/npis/1174629083) business and practice location has NJ zip code and area code but a state of NV
  • I think NV should be NJ
• 1073714358 (http://www.bloomapi.com/search#/npis/1073714358) practice location has NJ zip code but Louisiana address.
  • practice zip of 07012 should be same as business location zip (70121)
• 1750491288 (http://www.bloomapi.com/search#/npis/1750491288) business and practice locations have zip code of 00617 which is for Puerto Rico.
  • practice and business location zip codes should be 60617
• 1114183464 (http://www.bloomapi.com/search#/npis/1114183464) provider zip code is that of 33021 (Hollywood, FL) while the address and phone number map to (Hollywood) CA.
  • this issue seems to only be in the http://www.cms.gov/Regulations-and-Guidance/Legislation/EHRIncentivePrograms/Downloads/ProvidersPaidByEHRProgram_June2013_EP.zip file and not in the NPPES NPI database.
  • zip code should be for Hollywood, CA not Hollywood, FL
