[SVG] Stretchy characters that have no single-character versions should have minimum size #156

Stretchy characters with no single-character versions can be made too small. For example:

    <mtr>
      <mtd>
        <mo minsize=".75em" stretchy="true">&#x23DE;</mo>
      </mtd>
    </mtr>
  </mtable>
</math>

…ingle-character versions, and update the minimum sizes to be correct. Also improve placement of glyphs in CHTML stretchy characters, and add the missing fonts to the stretchy font list. Resolves issues #156 and #152.
