I supplied my PAUSE ID PATCH (all uppercase) to initially login but never received an email. Later, I returned and supplied patch (all lowercase) and received the email promptly.

Randy Stauner at blogs.perl.org also reported this issue: http://blogs.perl.org/users/mo/2011/07/exciting-updates-to-metacpan.html

This seems to be a problem with the mail server @cpan.org. They seem to be very aggressive regarding spam and metacpan sent quite a few mails out because a lot of people were signing up. We also added a SPF record, but we don't know if that did any good.

Actually, mattp just tried using his lower case PAUSE id and was finally sent his email as well. So it's either an issue of casing or of spacing out the activity.

the script doesn't send an email just blindly to [given id]@cpan.org but looks up the id whether it actually exists and then choses the first of the supplied email addresses. So it really doesn't matter which casing you're using. If you provide a PAUSE ID that doesn't exist you are redirected to an error page.
