Terminal not showing docker command output

Issue Type: Bug

Running with Ubuntu 18.04 remote. If I open a terminal and do the following none of the commands I run in the container show any output.

Docker shows output if run outside of vscode and if I switch to the output tab and back to the terminal the output is shown.
