Add a "Got it" close button to "reasons to disconnect help text" #4633

Came out of #4626

A couple of the "reasons to disconnect" cause subsequent "help" text to be displayed to the user. The help text is displayed in a modal dialog that has no close button. This is confusing, especially on mobile.

@ryanfeeley says to add a "Got it" button that closes the modal.
