Steps to reproduce the problem:

  1. Open the PDF in Firefox

What is the expected behavior?
The PDF is loaded and a form is shown.

What went wrong?
The PDF fails to load.

The same problem is occurring with the PDF from #4106, https://mail.gnome.org/archives/evince-list/2013-November/pdfNC0hkVW67a.pdf.
