Use a proper group name in 'contains'
The "ALL" group name should be the only item in 'contains'
argument. It seems to work like "ALLBUT" when followed by comma and
other group(s) but that's confusing, so I use "ALLBUT" instead.
Fix text formatting when it contains URL
This patch fixes the bug when if a URL is used within formatted text
(bold, italic, footnote and so on) it prevents the text from proper
highlighting because of double slashes in the URL (http://...) which are
recognized by vim as starting of a new italic region.
Fix URL highlighting
Some links have special trailing symbols which shouldn't be considered
as a part of the link. The explanation is in test_syntax.txt file.
Improved highlighting of links
Fixed a bug when a link beginning with "[[" could be closed by single
"}" or "]".
Added support for long links spanning more than one line in source code.
Added special highlighting for link control symbols (square brackets,
curly braces and bar).
Add image link formatting
Image links are complex links using a picture instead of a text caption.
They have the following format:
  [[URL|{{file|caption}}]]
where no }} or ]] are allowed inside curly braces. The previous code had
a bug when alone } or ] broke highlighting of such links. This patch
solves this bug.
Additionally I introduced a separate highlighting for the symbols ],},
and | when they are used for links formatting.
Multiline links are allowed.

thank you very much for your pull request. I'll try to get to it asap.

Added a possibility to enable/disable comment plugin highlighting
This is done because the comment plugin is not always used (and it may
be so a user has no permissions to add it even if he/she wants).
