Create new v8-4.5 branch with V8 4.5.103.24 #2509

Do NOT land on master

This is the new v8-4.5 branch with V8 4.5.103.24, branched from a recent master. This will deprecate the next+1 branch. This is basically a virtual rebase of the the next+1 along with an update to the latest V8 4.5 branch-head. For background see #2452 (comment) and #2452 (comment)

The `context_` is not initialized until the `CreateV8Context` will
return. Make sure that it will be empty (by moving away initialization
from constructor) at start, and ignore getter callbacks until it will
have some value.

Pushed a new branch https://github.com/nodejs/node/tree/v8-4.5. I will delete next+1 after waiting a few days.

Once I scrolled through the list of branches in the repo, I had a sudden distaste for the new branch name. v8-4.5 doesn't work too well when we have branches such as v3.x. Does anyone else have the same reaction? Perhaps we can rename it to V8/4.5 while it is still new?

No strong opinion. Slashes in symbolic refs can be annoying with some clients and/or actions, though.
