As the summary implies, this is a random and inconsistently occurring problem.
For your reference, I use Mozilla Firefox version 40.0.3, Windows 7 Ultimate 32 bit, NVDA 2015.3 stable and numerous websites can assist in reproduction of this issue.
For the moment, I shall try to demonstrate this issue using the example of the URL https://www.coolblindtech.com/check-out-voice-dream-reader/
After opening this website, jumpt to the 'Leave a Reply' heading.
With a reasonably fast speed (to increase chances of reproduction), press down arrow continuously.
It is highly likely that you will be placed in focus mode for one of the edit boxes - either E-mail Address or Name.
Note: I have ensured that the 'automatic focus mode for caret movement' check box is unchecked.
