Describe the bug
The placeholders of form elements are not written in a good meaning.
The grouped checkboxes/radios should not be wrapped in <div> tags.

Expected behavior
Change the placeholders with meaningful and appropriate texts.
Wrap the checkboxes/radios in <fieldset>/<legend>
