Voice Recording: Slow start causes duplicate sessions, microphone not released after transcription

What issue are you seeing?

Voice recording takes a very long time to start. Because there is no clear feedback that it has started, I clicked the button multiple times. Eventually recording began, but this appears to have spawned multiple background recording sessions.

After completing the transcription and submitting the prompt, Codex continued to hold the microphone — macOS showed it still in use. It seems the extra background recording sessions were never properly terminated, or the microphone connection was not released correctly.

What steps can reproduce the bug?

What is the expected behavior?

1. Voice recording should start promptly with clear visual feedback so the user knows it has begun.
2. 2. Only one recording session should be active at a time, regardless of how many times the button is clicked.
3. 3. After transcription is complete and the prompt is submitted, Codex should fully release the microphone so macOS no longer shows it as in use.
