With a direct zip unload on server(note: WINDOWS o/s), the result you're likely going to see is that the path is incorrect (typo in jscript).
Edit {root}\index.php

Should look like:

$path = rtrim(str_replace('\\','/',dirname($_SERVER['PHP_SELF'])), '/');
Notice the replace. Unix paths are forward slash. Windows are not. PHP returns O/s based path. Therefore you replace backslash with forwardslash, to properly load scripts in web context.

Had same issue myself about a minute ago. I believe that code would be compatible with Unix (repo assumed enviro), so should be an easy update for authors. Hopefully it's ok that I'm not making a PR on that.

P.s you should open IIS (assuming non apache), and use the Url Rewrite Import button to import htaccess rules to web.config rules. This must be done for both {root}.htaccess, and {root}\api\ .htaccess
It needs to route all requests to the index.php file.

Edit: Also do it for:

• {root}\api\vendor\slim\slim
• {root}\api\vendor\ezyang\htmlpurifier\maintenance
  (web.config import rules should be copy pasted to manually created web.config residing in that folder, or use the xml tag in root config)

Blank page for me came from the o/s path issue, but you may want to turn on debug output to see what it's doing (paste at top of index after the <?php :

ini_set('display_errors', 1);
ini_set('display_startup_errors', 1);
error_reporting(E_ALL);

Or if you can specify a .user.ini based on your server-level php.ini, that is better as you can specify error_log and get a log file for any php execution

I'd recommend using the attached web.config above, that is the complete file to be placed in the root folder (you could manually do it for each .htaccess, and put a separate web.config in each independent folder too).
Also make sure you have the IIS website configured to the correct public folder, and that rewrite module 2.1 is installed (link above).

Let us know if you get any debug output after that, may help identify issue if it's not the same as one above.

This is still an issue even on Ubuntu. After trying all methods mentioned in the open and closed issues I am still getting a blank page with the same 404 error on the console. The above error reporting code shows nothing. Anybody has this working?
