Rework cold key swap from scheduled to announce/swap

The idea is to rework the coldkey swap working as follow:

• you first announce your swap (with hash, as to not expose the destination)
• when the minimum delay has elapsed since announcement, you can call swap.

This is similar to the announce/proxy_announced.
