add severity and likelhood in the hazard record

Add the severity and Likelihood in the hazard record and check the generation of FHA

The tricky part is how to do it for different labels we have.
One way of doing it is a single enumeration that covers all possible labels. Another one is to have separate severity fields for each label set:
e.g., severity, ARP4761_Severity
