Textual instance model doesn't resolve referenes to mode transition instances

The textual instance model assumes that references to mode transitions point to the declarative model which they did because of a bug in the instantiation (#1096). They should be resolved to the corresponding mode transition instance, instead.
Names of mode transition instances should be changed to an identifier - without '.' in the name - to make this work.
