Maintenence: Remove old attachments

We have two systems for creating items attached to your avatar and it is difficult and confusing to maintain both.  We should stick with entities as the entry point for creating content in the world and remove attachments.  Care will be given to not break existing scripts by converting the properties internally.  May need to discuss/investigate permissions concerns.
