Digital Health Hack 2012 Winner.
GP Hop is an application which allows a user to enter their postcode to find a list of nearby practices, this used the NHS Choices web API to display information such as the location and user ratings. Further development would add NHS QoF data to give the user a more personalised choice.
Once the user has selected the practice they wish to switch to they are prompted to enter their NHS number, this would be traced using the national PDS.
The practice is then emailed and can choose to accept or reject the transfer, the system would then initiate a GP2GP transfer.
The national PDS, GP email and GP2GP transfer were all simulated during the hack.
