    Introduces new files needed to run on Windows Azure. File MSSQLStore provides SQL statements for MSSQL RDBMS. File SQLStore had to be modified in order to support MSSQL sqlsrv driver. Hence, migration to MDB2 PEAR package was done.
    In addition, session handling which uses database as a store was implemented because its better in Azure.

    This prevents a blank page with no error indication at all, if there
    appears to happen a fatal error when trying to require DB.php from
    the global include path (very bad practice!)
    Instead, using is_subclass_of should be enough, especially when
    using an own implementation.

    PHP OpenID libraries do not work on PHP 5.3
