Allow pouch to be called globally on a remote database #632

We cant use Pouch to call global functions on remote databases since location is tied to having a database name, right now the main issue is _all_docs, however going forward there is likely going to need to be support for a lot of these (sessions, tasks, global changes feed, config etc)

Taking a cue from nano, this could probably look like

var PouchServer = Pouch.use('http://example.org');

then

PouchServer.allDbs(....)
var db = PouchServer('dbname');

etc would all go to the remote server

var db = Pouch('dbname')

would continue to act as normal (local by default)

Closing in favour #848, instead of doing bit by bit API improvements I would like to get the whole API to a point where we are happy supporting all of it
