Looks good to me. Tested some basic operations with a local image - seems to work well.

Added all the necessary configuration to set up a self hosted demo with MSSQL as the source database.
Updated the nodejs demo backend in this PR
This should only be merged once the MSSQL changes in the powersync service have been merged.
