  1. Skip range ghosts in ColumnFamily.get_range()

This means that range ghosts (i.e., you get back a key,
but no columns) will no longer count towards the requested
row count and will no longer be yielded by the generator.

  2. Make CFMap.get_indexed_slices a generator

This makes it consistent with the behavior of
    CFMap.get_range, CF.get_range, and CF.get_indexed_slices.

  1. fix test README

Tests fail when using OrderPreservingPartitioner as it requires UTF-8
keys, and it's deprecated. Change to ByteOrderedPartitioner.
    All tests pass.

  1. 1. Fixed typo in type exception

    2. If using types, a type such that (not Val) is False will not by unpacked. For example, use an int type and put a 0 in.

  1. Close conn and decrement count on TApplicationExc

Although rare and generally indicative of a programming
error, the proper response to a TApplicationConnection
is to close the connection and decrement the ConnectionPools'
    connection count.
