Add ISSUE_TEMPLATE.md file

Lately, people have been opening a lot of tickets with questions that would not belong to the issue tracker. Since GitHub provides support for issue templates, let us add one with some guidelines/hints on how to properly open a ticket and where to go to look for general help.
