It seems that there are some discrepancies between the migrations in /db/migrations and what ends up in your migrations folder when starting a new project.

This seems to be caused by discrepancies between the files in /db/migrations and /pages/db/migrations (and possibly in other engines that are packaged as gems). The migrations in /db/migrate work, but those created by the generator do not.

First problem: Not the same amount of files.
/db/migrate has 21 files, a newly generated app has only 20 (I can't quickly determine which one is missing).

Also,
https://github.com/resolve/refinerycms/blob/master/db/migrate/20110329222114_create_seo_meta.rb
is not the same as
https://github.com/resolve/refinerycms/blob/master/pages/db/migrate/20110329080451_create_seo_meta.rb
The second file gets included when generating a new app, this causes a migration error of "unknown attribute: browser_title"
