Gem::Indexer update_index not loading gems #232

gem generate_index --update is a bit confused - it now tries to update the existing local rubygems:

root@gems [/root]# touch /gems/rubygems.org/gems/exvo_helpers-0.0.8.gem
root@gems [/root]# rvm ruby-1.9.3-p0@mirror do gem generate_index --no-legacy --modern --update -d /gems/rubygems.org/

this should only update one gem, the five are the installed local gems.

Initialize Gem::Specification in the same way when incrementally
updating indexes as in a full reindex.
