README: "Sinatra installs different `not_found` handlers for development environment." What does this mean? #610

This is a bit naive.

I was going through the README and couldn't understand this part. The statement is present one line above this: https://github.com/sinatra/sinatra/blob/master/README.rdoc#rack-middleware

Recently, there was an issue posted where the not_found module gets called twice in production and only once in development. Is this setting somehow related to that issue?

I would also be glad to see that sentence made clearer. I've always experimented lots of difficulties writing robustness tests (testing unhappy paths) for sinatra applications, most probably because Sinatra's error handling behavior changes in subtle ways depending on the environment, which is odd IMHO.
