When sqlmap is provided with a raw request from a file (-r switch) it should maintain the HTTP request headers order

urllib2 module treats headers in a non-ordered fashion. Passing headers in (e.g.) OrderedDict mode will just rewrite it into dict type:

    def do_open(self, http_class, req):
        headers = dict(req.unredirected_hdrs)

RFC 2616:

The order in which header fields with differing field names are received is not significant.
