When a module cannot be found, inform the user about the parent filename for tracking down a bad require path easier.
if module A has a node_modules folder with module B, it should be able to require B and have that work as expected.
No need to list the same require twice in the dependencies if it is the same file. The same name will result in the same file from any given source so just deduplicate the requires array.
