Country list for SurveyMonkey missing UK and Ireland #1415

I hope this is the appropriate place to log this issue - I couldn't find anything directly about the SurveyMonkey in the code but several issues talked about integrating it.

I noticed this when reviewing the results for https://starescomp.github.io/2018-11-27-standrews/ - but it applies to other (recent) workshops too, e.g. http://sbc.shef.ac.uk/2018-11-26-sheffield/ where I checked the "Pre-workshop Survey" and "Post-workshop Survey" (but did not submit a full result, aborting after reaching the country question).

The drop down is missing the Republic of Ireland, and has Great Britain rather than United Kingdom (and thus is missing Northern Ireland too).

I think this listing and keyboard entry of U and K likely explains two mis-entered countries of Korea, democratic republic of (South Korea) (which confusingly to me is sorted under S), and Virgin Islands (UK) (the only appearance of the abbreviation "UK") while most of the group picked Great Britain.

Expected Results:

Add: "Ireland"
Replace "Great Britain" with "United Kingdom (UK)"

@pbanaszkiewicz I'm keeping this open here since @kariljordan is working on it but this affects Survey Monkey only. It does not affect AMY.
