If the user has a shipping addy different from billing, the use billing addy checkbox should not be checked and both addy's should be displayed

If a user has different shipping and billing addresses, the my adresses view should not have the use the billing addy as shipping addy checked, and both of the divs should be shown with the checkbox unchecked. If it's checked, then the shipping addy should be repopulated with billing addy.

I consider that a bug because you can go to my addresses and update your billing address and inadvertently update your shipping address without any clear indication you are doing so.

Also, navigation links such as cancel/back to my account/etc. should be present.

Edit my addresses link should also be more prominent. No one notices it.

These are usability enhancements, but I think them to be important.
