Automatic Github issue body should include cluster info #446

I find myself repeatedly asking users for their cluster version, whether they're in Minikube, etc. Telepresence usually already has this info; it should be included in the automatic Github issue body.

Improve the crash reporter
- Capture more data in crash (fixes #446)
- Make it clear that crash data is not submitted automatically
- Catch the end of the log file on crash (fixes #466)
