These are the <a name="$ident"> tags in Pod::Simple::HTML and in <h# id="$ident"> tags in Pod::Simple::XHTML. RT-56572.
failures with versions of HTML::Entities older than 3.46. See RT #60450.

We can just let them be generated.

POD tag found inside a complex POD tag (e.g., "C<<< I<foo> >>>") is again parsed as a tag embedded in a tag instead of text and entities. The previous interpretation of `perldoc perlpod` was mistaken. (RT #55602 from Christopher J. Madsen) (reversed commit e1fe004)
Bump version to 3.13.
It seems useless and broken when used with ASCII or EDBIC output.
Fixed a regression in Pod::Simple::XHTML where '=begin html' blocks were incorrectly escaped rather than simply passed through unmolested.
This allows it to be used by Pod::Simple::HTMLBatch.
