Khottabun: Represent Thai Noi subjoins with PHINTHU.

* fonts/Khottabun.sfd:
  - Rename 'dummy' glyph at slot 0x200d back to 'zerojoin'.
  - Add detached 'dummy' glyph back at slot 0.
  - Fix "'ccmp' TN conjuncts" substitutions by replacing
    'dummy' with 'zerojoin'.
  - Add "'ccmp' TN subjoins" lookup for subjoins.
  - Detach subjoin glyphs from Unicode codepoints,
    rename them to uniXXXX.sub form,
    add "'ccmp' TN subjoins" substitutions as appropriate.
  - Add dummy 'uni0E98' glyph as required by subjoined
    THO THONG substitution.
  - Bump font version to 004.000.
