Uploading a file with an uppercase extension to S3 name returns the wrong filename on display #385

When uploading an image with an uppercase extension, the original filename is stored with an uppercase extension. However, the other sizes are stored on S3 with a lowercase filename. When the images are to be displayed, the uppercase extension is used for the S3 images, causing them to return an AccessDenied message (since they don't exist).

This issue occurs on the to_s method of an attachment when there is a default style applied. Test and fix is supplied here: https://github.com/reenhanced/paperclip
