bind_cols segfaults when the input list contains only NULL columns

bind_cols(list(a = NULL, b = NULL))

 *** caught segfault ***
address (nil), cause 'memory not mapped'

Traceback:
 1: .Call("dplyr_cbind_all", PACKAGE = "dplyr", dots)
 2: cbind_all(x)
 3: bind_cols(list(a = NULL, b = NULL))

Should probably fail instead of segfaulting. Possibly related to #1148
